I hereby confirm my participation in the 2026 Jesus Awakening Prayer Conference (hereinafter "JAPC"), to be held at Grace Ministries International (hereinafter "GMI") from Sept 3-5, 2026. During my participation in JAPC, I agree not to hold JAMA, GMI, its employees, or its administrators liable for any loss or damage to my personal belongings, or for any accidents or injuries I may sustain. I attest to my voluntary agreement to the aforementioned terms by affixing my signature below. By signing this document, I authorize JAMA and GMI personnel—including employees, staff, and faculty members—to transport me to a physician or hospital in the event of a medical emergency; furthermore, I consent to the administration of necessary medical treatments or emergency surgery, including X-rays, as deemed appropriate by a physician. By participating in this conference, I grant JAMA and GMI permission to utilize photographs of my activities taken during the conference period across their various communication channels (including websites, brochures, and social media).
